Do you have what it takes to be the ultimate SURVIVOR? Here’s your chance to be part of the adventure of a lifetime.

 

CBS 2 Chicago and Six Flags Hurricane Harbor are holding an OPEN CASTING CALL on Wednesday, June 15, 2005, from 6AM to 10AM.

To apply for SURVIVOR at the OPEN CASTING CALL, you must download and complete the application form and bring all three things below.

 

Complete application packages must consist of the following:

 

1. Completed application forms (including signed Release and Waiver at the end of the application).

2. Proof of a valid U.S. passport (or proof of having applied for issuance or renewal of such).

3. A passport-size photo.

 

Applications will only be considered if they are complete.

 

CBS 2 personnel will allocate two minutes of tape to each applicant. You need to tell us everything we need to know about why you would be the perfect SURVIVOR. Remember, personality counts!
